You have been specifically selected by me to participate in a valuable Medicare benefit called the Premium Wound Care Program. You qualify for this program based on your recent inpatient stay.

You may ask, “Why is this so important?

It's important to me personally to help you continue to heal after your last inpatient episode.  I understand that chronic wounds can not only cause physical pain and discomfort, but can also be difficult to manage. My team of experienced nurses are dedicated to helping prevent another inpatient hospital or rehabilitation stay.

Wound Care Management
  • A member of my team will call you each month at a time that’s convenient for you.

  • You will be able to discuss your wound healing, changes in your wound, preventing future hospitalizations, and any changes in your medical conditions.

  • I receive a progress note on your healing; this information will help me keep up with your status and provide recommendations in a timely manner. 

  • Your progress note becomes part of your medical record.

  • This is a virtual visit in your in your home and does not require a visit to an office.

  • This program is individualized to you and is not related to any calls you may receive from insurance companies.

  • This is a great way for me to help you reduce your risk of future hospitalizations and staying healthy.

 

Your participation in our new Medicare covered Premium Wound Program is strongly encouraged.
